The Improving Care for Lower Extremity Joint Repair MVP is comprised of measures most often completed by doctors in the orthopedic specialty. What is an MVP?

MIPS Value Pathways (MVPs) are a MIPS reporting option that falls outside of traditional MIPS and the APM Performance Pathway and is specialized to meet MIPS requirements for a designated specialty. MVPs can be thought of as a tailored reporting option that aligns with a specialty. They are designed to reduce reporting burdens, create meaningful comparisons, and provide relevant feedback to participants.

How are MVPs scored?

Scoring occurs like traditional MIPS, however, the Quality score is based on the total of r4 measures instead of 6. The highest 4 measures will be calculated and then divided by 40 (10 total points for each measure). In traditional MIPS this calculation is based on 6 measures and divided by 60. The resulting number Is divided by 30 to determine how many points out of 30 the practice will receive for that Quality Category.

If the practice is only reporting on Medicare Part B Claims and there are less than 4 Medicare Part B measures, the score will be calculated based on just those measures and the numbers adjusted accordingly.

For small practices (15 or fewer clinicians), the same exceptions, re-weighting and bonus points are available.

Here are the specific MVP scoring and reporting details for the 2024 performance year:

Quality (30% of the total MIPS Score)

  • Measures are scored against benchmarks and receive points on a scale of 1 - 10 points.
  • If required measures are not reported or do not meet case minimums or data completeness requirements, 0 points are received.
  • If less than four measures apply to a clinician in an MVP, report all measures that apply.
  • Small practices will receive a minimum of 3 points for all measures and 6 bonus points.
  • Must report 4 of the Quality measure options; at least 1 must be an outcome measure (or a high priority measure if no outcome measures are available.

Cost (30% of the total MVP score)

  • Measures receive points on a scale of 1 - 10 points.
  • If no cost measures apply, the component will be re-weighted to 0% and redistributed to other components.

Improvement Activities (15% of the total MVP score)

  • Medium-weighted activities receive 20 points each; High-weighted activities receive 40 points each.
  • Must report 1 of the following 3 options:
  1. 2 medium weighted improvement activities
  2. 1 high weighted improvement activity
  3. The IA_PCMH activity

Promoting Interoperability (25% of the total MVP score)

  • Will be scored as they are in traditional MIPS; Bonus points are available for small practices.

Population Health

  • Must select 1 measure at the time of MVP registration
  • No data is needed for this measure (calculated using administrative data claims).
  • This measure will be excluded from scoring if the measure doesn’t have a benchmark or meet the case minimum.
  • Subgroups will be evaluated at the affiliated group level.
